Short-Term Stay After Hospital Admission

Patients that have been admitted to a hospital for a minimum of three nights may have the option for a short-term stay in our center to receive 24-hour Nursing care and Physical, Occupational and Speech Therapy up to seven-days each week. Patients may also request a short-term Respite in-patient stay for periods of a few days to several months.

Patients that require a permanent in-patient stay may be admitted from the Hospital, Assisted Living Facility or Home after receiving an admission order from their primary physician.

The entire staff was joyful and comforting. From the first face you see at the sign in desk, check in, activity director, nurses, patients, laundry, kitchen, pt , speech, a very pleasant atmosphere and smiles all around. No gloves, no wipes, no wash clothes, couldn’t find a bedpan and kept trying to put a diaper on me because they didn’t want to have to deal with me using a bedpan! I had been at Wake Med for a month and not one problem this place is HELL ON EARTH!!!! I wouldn’t feed the food to a stray dog! I hope and pray for the people that are there and can’t speak up for themselves, some of the staff were nice while others could not care! They are definitely understaffed and I could go on and on but will not, if anyone would like to contact me please feel free to I pray for the ones there!

My response was,!She’s been sitting in dialysis for five hours for five years and never had a bed sore .when I expressed this to the staff, they said well OK, yeah we’ll make sure we do a little better and turn her more. SMH .In addition went  from very verbal to nonverbal! if you love your loved ones keep them out of here . I also asked about the laundry scheduling as they took her dirty clothes, filthy clothes and folded them and put them back in the drawer like they were clean they express to me they do laundry when they can?The care here is absolutely horrible!!! if you love your loved ones, please don’t bring them there.

                            [review] => I read all of the previous reviews and have to admit that I'm a bit puzzled about some of them. I guess that the administration and staff have recently changed because my visits to see my best friend were always extremely positive.

As an RN with over 30 years of experience, I can honestly say that if I wasn't retired, I'd gladly work at Raleigh Rehab. The nurses and CNAs are very kind and skilled; they also work well together as a team.

My friend transferred to RR from a nightmare of a nursing home in Durham. She'd fallen 3 times, once breaking her shoulder. The other times that she fell, she sustained closed head injuries. She was emaciated because no one helped her feed herself and unable to walk, due to PT not being provided for her in Durham.

I saw an immediate difference in her after she transferred to Raleigh Rehab. She was smiling and happier than she'd been in months. My friend told me that she felt safe at RR and that all of the staff members were very kind to her. They spent time talking to her and didn't rush out of her room, which made her feel that they really cared. She wasn't left in dirty diapers or in need of pain medicine because the call bells were answered in a timely manner at RR.  Additionally, they helped her bathe, brush her teeth, and eat. She reported that the food was good, as well.

My friend's room and bathroom were very clean and her trash was emptied regularly. The room was large, airy, and sunny which made it quite pleasant.

I was very impressed by the nursing staff. When my friend asked questions about her condition, care, or medication, she was given honest and direct answers. The nurses obviously were familiar with her and had actually taken the time to read her chart. They kept her husband updated on her progress and he felt comfortable returning to work, knowing that she was happy and in safe hands.

I can't begin to tell you how poorly I slept--and how much I worried about my dear friend while she was in Durham. Afterall, so many of her health issues were a direct result of the neglect and extemely poor "care" that she'd received at the nursing home in Durham. Once she was comfortably settled at RR, though, I felt secure in the knowledge that she was receiving competent care from their kind and empathetic staff members.

I'll enthusiastically recommend Raleigh Rehab to anyone looking for a nursing home/rehab facility in the Triangle.

                            [review] => They do not deserve any stars due to low staffing, patients going over 2 hours of not being turned, staff have to work their unit plus another unit to keep the shift/units going, they have too many roaches in this facility the patients are not cared for as they should be. My loved one was admitted there for therapy from suffering from a stroke and now dementia and within four days from February 3, 2019 to February 7, 2019 developed bedsores I have them on video and a gash in her left thigh from them using too little diapers and pulled them too tight and was wet and in stool for hours. That’s how pressure ulcers/bedsores developed with not being turn every 2 hours. And my love one was left in the room alone with her medication still in her mouth alone. Which could have lead to choking and possibly aspirating or even worse and they were informed to crush the medication. My love one door was completely closed when the situation occurred thank god I came when I did. The nurse did not wait to see if the meds were swallowed. The nurse failed to do a mouth check after giving my love one the medication I talked to the DON (Director Of Nursing) about the situation he stated “ we have to do a little bit better”. I stated “no that’s an understatement you all have to do better than what you all are doing here not only to my love one but every single patient in here you’re quick to accept them because you all need the funds to run the facility but you don’t care about the treatment of the patients and that’s not fair at all to them or their love ones. And I have proof on my phone by video of the roaches and the nurse admitting she did wrong and to state it was considered a medication error on her behalf”. And I mentioned to the DON about calling the state on this facility he got so dramatic stating while flaring his hands “call whoever you want we tell everybody that’s your right do what you want to do it’s okay with me”. Thank you to the Director Of Nursing name Boger (name on his badge) for giving me permission to do so with my proof on video of this awful facility. I wouldn’t dare send my worst enemy nor sick dog to this facility ever they will not make it in there far as medical treatment at Raleigh Rehabilitation Center 616 Wade Avenue Raleigh, NC. By the way the good reviews you all read really and truly are from the staff that work there not guests/visitors/patients.

Cleanliness could be a little better. The staff is very nice and they seem to be very pleasant with my mom. They have a TV there. That's pretty much what she does. With Alzheimer's, she doesn't do a lot. It's in the middle of Raleigh. So, there's a bunch of stuff out there. They have all kinds of restaurants there and hospitals.

Being from out of state, I was nervous about choosing a place for him. I called for information and was greeted very courteously. They suggested that I visit and take a tour. I was met by Miss Jasmine who took me through the facility and the floor my father would be staying on. I felt very comfortable sending him there. He felt comfortable, safe, and has only
Good things to say about the staff on his floor. He was only there for 12 days due to his insurance. He received 6 days of therapy- PT and OT- as they don’t always do therapy on weekends. He even got therapy on Thanksgiving day. They are very liberal with visitation. I was able to come and go as I pleased. My only issue was that sometimes at night I had to go back upstairs to ask to be let out the security door.

Miss Debra was dads case worker and was very helpful regarding his rehab plan, discharge plan and setting us up with home therapy.

This is an older facility with excellent staff. My only complaint is with housekeeping. His room was supposed to be deep cleaned before he arrived and the bathroom was filthy. I had to wipe down the sides of the tub  and used paper towels to clean the floor. Otherwise, dad was very happy with his stay!

She lived on her own, drove and loved being apart of her church group.  She fell on her way to exercise at our  senior center. During the 2.5 months at Raleigh Rehabilitation Center my mother was severely neglected.  Per the request from the doctor on staff she was put on a dietary schedule, physical therapy to help her stay mobile.  The lack of care at the facility so many things went wrong.  Mom passed from a stage 5 bedsore on her bottom.  Please keep your family safe.  I couldn't.  😢.

STAY AWAY! Raleigh Rehabilitation Center is full of incompetent staff. Due to Raleigh Rehabilitation Center my mom was taken to the hospital with a stage 4 ulcer/bed sore with bacteria to her tail bone. She is currently on her death bed.  Back in April she was enjoying life and living independently, driving and visiting my family. Now all of this is only a memory.   DO NOT COME HERE!!!
Mom passed away 7/2022 from her bed sore after being at Raleigh Rehabilitation Center from April to late June.  Her stage 5 bedsore caused her such pain.

                            [review] => Raleigh Rehab cheated me.  They received Medicare payments for my late husband's stay there AND collected payments from me too.  

My husband ran out of Medicare coverage for skilled nursing facilities last Fall.  How Raleigh Rehab was able to bill Medicare is something to be explored.

Since I paid in advance for my husband's stay there I am still waiting for reimbursement from them because he was moved to hospice in the middle of the month.  Raleigh Rehab said they'd "Look into it." but I haven't heard anything from them even though six weeks has passed.

On Monday I will turn this whole mess over to Medicare.  I feel assured they will investigate and take the appropriate action.

Some of us are almost finished with quarantine,  the facility decides to put other patients with covid in the same at the Directors orders. We were never advised that they were going to do that. It would of been nice to be informed.  But they don't Caras long as they can get any new patients in. According to the Director she is doing everything according to CDC guidelines. I feel that my rights and health has been ignored at the expense of money. My honest opinion of Raleigh Rehab is the Nursing staff is good. So are the therapy  department.  However both the Kitchen staff and The Director both are poor at best they hurt the facility overall both needs to be changed.

I was transferred here after major surgery for a broken femur and spent 7.5 hours alone in a room with none of my transferred heart, pain and other meds administered. Finally had Instacart deliver, food, water, and Tylenol. After asking for help from staff multiple times then falling, I had to call 911 from my cell phone to be transported back to hospital ER. DON'T GO HERE!!!
None of the staff (that I barely saw) wore gloves of any kind. The "house" doctor came in to show me how to use the television, then looked at my incisions with gloves he took out of his pants pocket!

                            [review] => So let me just start by saying that having a ill parent can take a toll on a family and especially a toll on a single child who is trying to make the best decision they can when it comes to the care their parent may receive when they become ill and need to be placed in a facility. With that being said trying to get anything done, questions answered from this facility is absolutely horrible. I read the previous reviews and I am trying to figure out how anybody would give this facility a 4 or 5 star rating. I will admin there are some social workers, and front desk employees whom seem to care about their job, but the nurses NO. Every time I came to visit my father they were sitting at the desk the hall would be lit up with call lights and they simply wouldn't answer them. My thought was wow what if it was an actual emergency how would they respond?? My father would holler for nurses to come and they would simply shut the door, which infuriated me, because if he is hollering he needed their assistance in some kind of way. Their solution was to have him evaluated by psych, instead of trying to assess his needs because they did not want to deal with him. I pulled my father out of this facility and took him home, and after he was discharged....no response from the director of nursing on his after discharge care, he needed physical and occupational therapy, and also a prior authorization for him to get his prescription filled. I called the director of nursing several times to try and get this resolved no response. just inconsiderate and unprofessional. My advise avoid this facility and if your loved one is placed here be prepared to check on them consistently because God knows they will be neglected.

She had been in Wake Cary from Dec 8 till Dec 31, 2021.  8 days later she was back in hospital with Covid 19 and septic from a UTI and unresponsive.  She has also lost 24 lbs. in these 8 days. My Dad has been to multiple Skilled Nursing Facilities for rehab in the last year, and suffice it to say, Raleigh Rehab is in an entirely different class than any of the others. My Dad's typical report for any given facility, on an average day, is just a litany of complaints (and often rightfully so.) But at this facility, he has had almost nothing but compliments. The few issues he has had, have been handled quickly and efficiently.

What really makes this place is the staff, which is is incredible. For the most part, our typical experience at nursing facilities have been staffs that are generally curt, lethargic, borderline apathetic, and difficult to get ahold of (sometimes even ghost-like.) The staff at Raleigh Rehab is the exact opposite in all regards.

The day of admission I was met with an avalanche of employees (in a good way,) giving me the lay of the land, explaining treatment plans, therapy goals, policies etc, and answering any and all questions we had. Every employee I have encountered has been energetic, professional, and hard working. And persistently so. A very refreshing departure from the norm at many of these facilities, where most employees seem like they'd rather be anywhere else.

Gotta call out Jackie, the admin, who is an absolute superstar, handling everything from administrative issues like scheduling appointments, visits, or policy discussions, all the way down to checking in on my Dad for no reason at all, wheeling him around the facility, running things up to his room etc. She is a Jackie of all trades. I do not know where she finds the energy. She is always extremely easy to get ahold of, and very reliably handles any issues we run into.

Danielle, the business manager, has provided (for the first time in my experience,) clear, and decisive answers to many questions that have gone largely unanswered by just about all previous business management and social worker employees at other facilities and even hospitals we've been to. I cannot tell you how refreshing it is to have clarity and confidence on some of the very intricate processes we've had to navigate (like applying for Medicaid, for instance.) It's obvious she is very experienced, and really knows her stuff.

PT and OT are incredible. Most SNFs my Dad has been to, the therapy is hit or miss, and pretty minimal. Somedays he gets it. Some days not. Usually it entails simply going through the motions with no clear goals. Meanwhile at Raleigh Rehab, it's not an exaggeration to say that therapy is their top priority (given it's the reason he's there,) and everything else is subservient to the therapy schedule (short of emergencies or critical medical appointments.) Literally within 20 minutes of admission, I was already having an extensive conversation with Felino (the PT-ist) about the goals and the plan. And since then, the therapists have kept me informed and updated with specific and quantifiable goals, progress reports, and treatment plans.

The nursing staff has been excellent. Competent, friendly, knowledgable, and attentive. And the nursing manager, Angie, really knows her stuff, pays close attention to patients, and is very good with communication.

On that note, one of the most standout things about this facility so far is that the staff is not only very easy to reach, but is actually proactive about keeping my family up to date and informed. It's so nice not to have to chase people down, and to always be in the know.

The three or so weeks my Dad has been here have been a fantastic experience, and night & day compared to other facilities he's been to. We've grown to trust the staff (didn't take long, ) and we enjoy peace of mind that my Dad is in good hands, is comfortable, safe, and dare I say, generally happy. Definitely not something that comes easy for him in Skilled Nursing Facilities. I could go on all day, but the takeaway is, if you have a family member looking for skilled nursing rehab, and you are given a bed offer at Raleigh Rehab, I'm telling you, take it.

They do a great job at keeping us up to date on any changes with my Dad. They are also incredibly responsive, allowing my sister and I to schedule Compassion Visits with my Dad during this tough time. Everyone we speak to is warm and welcoming, and extremely supportive when it comes to navigating options and support for my Dad.

This facility is trending in magnificent ways!  The location is in the heart of an upscale neighborhood, with ample parking including covered parking accommodations.  I am most impressed with the current staff.  The new Administrator, Sekeithia Jones has an made some dramatic positive improvements since my last review six months ago.  During her three- four month administration, I have seen increased patient engagement, this administrator is hands -on, she assists the staff and patients by helping to feeding patients, she has increasing family participation by hosting Town Hall Meetings, and reducing agency staff to ensure committed continuity of care! The Staff Development Coordinator EJ provided staff and residents with a mobile in-house Flu Shot Clinic.  The Activities Director Linda exudes enthusiasm for the residents by ensuring each resident feels cherished and motivated in the various creative activities provided daily.

The collaboration between Bethany of Human Resources, Staff Development and Administrator are on point. I see more smiles and camaraderie among the staff.

This facility utilizes EMAR Electronic Medication Administration Record, has highly trained nurses that competently work to their full scope of practice and can successfully treat complex surgical wounds.
Patients and Residents are seen by a physician within 24 hours after admissions and those requiring ancillary care are provided weekly visits by board certified Wound Nurse Practitioner, Psychologist and Psychiatric.  Social Work assessment are conducted, and both the long term and short-term patients are provided with vision, dental and podiatry care as well as safe proper care plans for those being discharged.  The Therapy staff PT, OT and SLP is amazing!

I continue to stand by my recommendation:
Should a loved one require assistance via therapy, skilled nursing or long-term care, this facility has the caring knowledgeable dedicated staff, clean and well-maintained space, sophisticated equipment and technology to exceed expectations!  It is truly evident that the entire staff from the Administrator to Housekeeping considers each patient/resident important by answering call bells quickly to reassure patient/resident they matter most!

My mother was sent here after a brief time in hospital to finish a course of IV antibiotics and to keep her strength as she has MS and is wheelchair bound. I was greeted by a patient (or resident since they have both) in a wheelchair trying to get onto the elevator, she was grabbed from behind and screamed at and told she wasn't going anywhere.  Patients/residents were in their bedrooms or in the hall asking for help, aids were ignoring them - it was like a scene from the movie one flew over the cuckoo's nest.  My mother had her light on and had been waiting for someone to come in to give her some pain medication, she was in a state and had been dumped into the room and left for ages so she was uncomfortable and disheveled.

I had her moved to another floor but the staff really do not care and neglect is rife, I have had to visit many times a day because it is so worrisome. Just looking at my mother I knew she was running a fever, I had to chase them down to take it and yes, she did have one. This has been an exhausting experience for me too because I am in extreme pain from the effects of Pneumonia that I am trying to get over but it has been a full time job, telephone calls and visits and the care is just awful. My mother is leaving today, I am having a fit because the longer she stayed the weaker and thinner and sicker she was becoming. When I entered her room to pick her up she was on her chair dropped over looking like she had passed out, she was weak and unable to drive her own electric chair.  Normally I would have refused to move her but I just want her out of this place so I managed.  Do not let a loved one enter this place, it should be shut down.

Apart from roaches infestation, bed bugs, the nasty attitude from staff members, they are feeding this people with expire food. The good reviews you see here is from staff members that get prices from HR if they do good reviews. Took us 15 minutes to find out this and more. Why the health department hasn't done nothing about this situation?

                            [review] => My family member went in to Raleigh Rehab (RR) after suffering a stroke (accompanied with other issues as well). The  level of care and therapy required far exceeded home care. My family rotated daily visits, through out every day of the week for 3 yrs. The first year was okay as staff and quality of care was acceptable, then RR changed management and staff... everything rapidly declined. The unfavorable things people are posting here are true ten-fold. It’s horrible. I will spare you the details of the unforgiving experience my family member endured there... but can tell you, the final days there were painful and completely preventable. My family member was ultimately left unattended with the bed set on the highest setting, the fall mat removed and guard rail down by a CNA that walked away. Unable to walk, my family member fell face first on the cold hard floor... no telling how long they laid there in excruciating pain... only to be found by someone who happened to pass by and noticed them on the floor. 3 days later she died. The staff acted like they didn’t know anything, they turned away from the family. The lady who runs the place assumed no accountability for what happened, making pathetic excuses when my family confronted her. She had the nerve to say the nasty bed sores  (due to the negligent staffing care) would have taken her out sooner than later. If that wasn’t enough... the CNA who walked away creating significant fatal risk ... kept his job!! Total death sentence.  No scruples whatsoever. I can’t believe they get away with the obscene things they do. If I could get justice for my family member, I would without hesitation  ... but RR banks on the outcome of families walking away, trying to preserve any dignity left of their loved one, I’m sure. I can share what I know to be factual though. Take what you will from this. Your loved ones deserve far better... I know my family member sure did.

Some staff try to do their duty, but the vast majority of the staff lack of empathy and should not have any business in taking care of patients. My family needed to remind them what to do, after some safety measures were in place. My family member suffered a stroke and needed strong rehabilitation services. We regret our loved one was admitted there. This by far is the worst facility I have visited, counting that I am knowledgeable about the business of rehabilitation. It is difficult to conceive how facilities like this can remain open. The medical staff ( Nurse practitioner) was evasive and never responded to the family requests to answer some questions. The medical director was evasive, and evidently instructed staff to avoid calling him. It seems that this facility  serve medical indigent population and the routine treatment is at best mediocre and inhumane. Please pay attention to your family member closely if happens to be admitted there. It is not uncommon to find centers like this understaffed, but when the quality of the majority of the staff is poor , it  makes it more dangerous to the loved ones. They topped their incompetency sending my loved one to the hospital without notification to the family and several hours later we receive a call from the hospital that my family member suffered two cardiac arrest. Their careless attitude after I called the staff to ask questions about the incident convinced me that  is the way they operate.

The staff constantly tells patients and family members that they are understaffed and cannot provide proper care. They do not answer the call button in a timely manner but sit at the nurse station and listen to it beep without checking on the patient. I purposely hit the button and left to see what would happen. There were 5 staff members standing around when I teturnd an hour later while the patient sat soiled for over an hour and when they were confronted they all seemed surprised and pretended they didn’t know. They often wait for one nurse or nurse assistant to care for patients as if they cannot assist because “it’s not their job.” I would recommend you take your loved one elsewhere if you have ANY say so! There are a few staff members that appear to care but that’s the exception.  If I could give it a 1/2 Star I would

They were very understaffed when it comes to the aides. My uncle is incontinent and they didn't change his diaper in a timely manner. The staff was very friendly and were nice people, but because there's not enough of them, and the poor management of the community, they're ineffective at doing their jobs. However, the therapists were awesome, the PTs and OTs were excellent, and they did great with my uncle. The place was pretty clean, but chaotic due to being understaffed. The food was fairly good.

                            [review] => My mother has been in Raleigh Rehab's long-term care area (nursing home) located on the 3rd floor for 10 rough months.  My mother suffered 3 major stokes and is paralyzed, unable to speak well, and requires a high level of care.  She's very coherent and understands what's going on around her.  A few people on the nursing staff are very helpful and kind to her.  Most have the wrong demeanor for this type of work and are mean and ignore even their patients at times.  They seem short-staffed and stressed.  
My mom went without having her teeth brushed for over 2 months causing extreme gum decease.  She's gone nearly 2 months without a full shower.  She gets urinary tract infections on a regular basis due to not being changed.  She's in pain a lot of the time because they don't manage her medications well.  Trying to communicate with their admin team has been horrible.  The more I've tried to complain, the more they ignore us. 
Before you admit anyone you care about into their long-term care program make an unannounced visit.  Tour the 1st and 2rd floors first.  Then take a tour of the 3rd floor and notice how it looks, smells, and the demeanor of the staff.  Compare what you find to other facilities in the area and come to your own conclusion.

If you have a choice, take your loved one someplace else, as this place is horrible.  My father was here for three weeks during which time he got aspiration pnemonia twice.  They do not give patients their meds until you complain about it.  You really need to take your loved ones care into your own hands if they come here.  One evening I found food that had been in my fathers moúth for over an hour because he could no longer chew or swallow solid food.  Its no wonder that he aspirated.  We had to tell the staff to put him on soft food.  Every night when I came to visit I would see patients wandering the halls looking lost, confused and lonely.  The staff did not seem to care. On his last day here, they decided to send my father back to the hospital as he was running a fever and his heart rate was high, my husband and i got the call, jumped in the car and on our way past the rehab facility we saw the ambulance was still there...so we beat them to the hospital.  I believe in my heart that this facility contributed to my fathers demise due to their shoddy care.

                            [review] => Is it a nursing home, rehab from a hospital stay or an asylum.  After this visit, which is my third (due to PSP Progressive Supranuclear Palsy), I am not sure. Judging from the sounds emanating from some the rooms at night and sometimes during the day; I would say an asylum, but one of the long-time residents called it a nursing home. In any case, the employees, for the most part, are great and very attentive to the needs of the patients. However, some of the patients require more care than the employees know how to give.  I think the administrator(s) need to decide what kind of facility they want and stick to it. I do not think there will be a fourth visit, even if I need it and they would have me and if you are looking for rehab from a hospital stay, I do not recommend it.

The nurses were very helpful and concern of his needs and made sure they were met in a professional manner. The nurses learned all of our names within a week. They were constantly assisting him getting him stronger and stronger. If it wasn't for those people my cousin would not have gotten his strength back as quick as he did.

Everyone at Raleigh Rehab was so nice and welcoming. They all worked really hard at making sure that everyone staying or visiting was welcomed and included. The entire staff was joyful and comforting. From the first face you see at the sign in desk, check in, activity director, nurses, patients, laundry, kitchen, pt , speech, a very pleasant atmosphere and smiles all around. We felt heard and my mom's needs were met. A few went above and beyond what was required and my family and I will be forever grateful. Keep up the great work
